The goal of this course is to train case managers about patient outcomes.

A case manager's primary goal is to facilitate the clients return to their optimal health and function. This goal is achieved via a patient-centered plan of care with a focus on patient engagement, patient education, and care coordination. The use of evidence-based guidelines ensures that optimal outcomes are achieved (CMSA, 2022). The impact of a quality case management program is seen in favorable outcomes across the healthcare spectrum. Transitions of care, readmission reductions, preventative care, and quality measures are examples of outcomes that case managers significantly impact. The case management standards of practice serve as a guide for the case manager (CMSA, 2022).

After taking this course, you should be able to:

  • Recall ways that case managers impact the outcomes of the Triple Aim.
  • Identify models of care that affect case management outcomes.
  • Indicate the role of case managers in preventing hospital readmissions.
