Growing evidence and awareness in the healthcare field show that a small percentage of people utilize a very large percentage of behavioral and physical healthcare resources. These high-end users often have multimorbidity and experience a variety of crises that can result in frequent use of emergent and high-cost care. Furthermore, this population may have barriers to accessing community-based health services that would support stability. Case and care management services, including crisis prevention planning, are critical to meeting the needs of this population. These services ensure that a risk reduction approach and crisis prevention planning are used throughout the course of intervention. The goal of this course is to  provide nurses and social workers working within payer organizations with education around crisis prevention and planning.


After taking this course, you should be able to:

  • Identify critical junctures in the course of intervention where crises are most likely to occur.
  • Develop a care plan that includes risk monitoring and crisis prevention planning and strategies.
  • Identify protocols care teams can use to best ensure continuity and care, member safety, and risk reduction.
