Preventing Blood Incompatibility Errors

Transfusions are a life-saving intervention, but transfusion errors, particularly errors involving ABO incompatibility, can be deadly. Three actions that have frequently been a contributing cause for ABO incompatibility errors are incorrect labeling of the blood sample for crossmatching, incorrect interpretation by personnel in the blood bank, and incorrect identification of the patient prior to initiation of a transfusion. Laboratory staff and nurses who are involved in the administration of blood products must strictly follow facility policies to ensure that they give the correct blood product to the correct patient.

Every Nurse Is a Risk Manager
Every time you complete an occurrence report for a near-miss medication error or review a clinical policy and procedure with the nurse manager, you are involved in a risk-reduction effort. Healthcare risk management is the process the organization uses to identify, evaluate, and manage risk and potential liability exposure. The objective is for adverse outcomes to be limited by establishing systems to prevent or identify errors before they affect care, and in the event errors occur, to mitigate loss exposure. This module will reinforce and strengthen the understanding nurses have of risk management in healthcare and suggest risk reduction tools to mitigate exposure to professional liability.

Importance of Nutritional Status in Pregnancy Outcomes
The most important controllable factors in pregnancy outcomes are prenatal medical care and adequate nutrition during pregnancy. The health and eating habits of the mother directly affect the fetus and the pregnancy’s outcome. In a country such as the United States where the food supply is plentiful, women should be well nourished. It’s disheartening to realize that malnutrition and poor eating habits still lead to maternal, fetal, and neonatal complications. Not only are there short-term complications for maternal health, but the child may experience chronic diseases later in life based on the mother’s prenatal diet and its impact on the mother and the baby’s epigenetics. Learn what nutrients are necessary for a healthy pregnancy and why nutrition should be a focus for preconception and prenatal care.
