Recognizing and Treating Five Shock States

Hypotension (systolic blood pressure < 90 mmHg) is often considered the defining hallmark of shock, but it can also be a late or deceptive sign. Therefore, other signs and symptoms must be identified early to detect and respond to a patient’s deteriorating state before irreversible shock ensues. The pathophysiologic process of shock is complicated, which makes nursing management challenging. An understanding of the pathophysiology, aspects of diagnosis, and effective interventions are crucial to patient survival. Knowledge of the different states of shock and evidenced-based interventions is important for nurses as they play a vital role in promoting timely detection and treatment. Such knowledge can help the clinical nurse promote care that is based on the latest guidelines.

Team-Based Healthcare Improves Patient Outcomes

Team-based healthcare is provided by two or more people who represent different professions with the common goal of improving the well-being of a patient. Outcomes such as relief of pain, improved access to healthcare services, early recognition of treatment failure, or other scenarios may be improved with interprofessional collaboration. The delivery of effective team-based, patient-centered care should be tailored to the population served and needs of those individuals. This module serves to outline the components of team-based care and examples of this care in different settings.

Pursuing a Healthy Weight

What is a healthy weight? Multiple factors impact our body weight and size, yet as healthcare providers, we often focus more on the scale readout rather than the behaviors that influence our patients’ weight and health. This module discusses the psychological and physical issues facing those with obesity, along with the impact of chronic dieting and weight bias on health. It also provides tools, such as tips on motivational interviewing, non-diet health goal setting, for the provider to use in clinical practice, and briefly reviews traditional weight management guidelines.

Excellence Series: Transformational Leadership
The complexity of the healthcare environment results in multi-tiered levels of leadership and relative autonomy for leaders to use their preferred style of motivation. Research has shown that not all leadership styles are equal, and there is a strong correlation between employee satisfaction with leadership, agility with change, and performance metrics (Peng et al., 2020). Transformational leaders are those who merit trust and respect by their employees by both developing the careers of others, achieving performance benchmarks, and upholding the vision of the organization (Dumdum et al., 2002).

The goal of this course is to aid nurse leaders in acute care settings in understanding the role of transformational leadership in healthcare, so they can lead their staff most effectively in  meeting the demands of a changing healthcare environment.

Every Nurse Is a Risk Manager
Every time you complete an occurrence report for a near-miss medication error or review a clinical policy and procedure with the nurse manager, you are involved in a risk-reduction effort. Healthcare risk management is the process the organization uses to identify, evaluate, and manage risk and potential liability exposure. The objective is for adverse outcomes to be limited by establishing systems to prevent or identify errors before they affect care, and in the event errors occur, to mitigate loss exposure. This module will reinforce and strengthen the understanding nurses have of risk management in healthcare and suggest risk reduction tools to mitigate exposure to professional liability.

POLST: Planning for Life Endings
More than two decades ago, the Patient Self-Determination Act promised more patient involvement in end-of-life treatment decisions. Unfortunately, studies have shown that current forms of advance directives have not resulted in healthcare providers' complying with patients' end-of-life choices. To address this problem, the physician orders for life-sustaining treatment (POLST) model and form are now used or in development throughout the United States. POLST documents medical orders based on a patient's current health status regarding CPR, medical interventions, and artificially administered nutrition. The POLST form is highly visible and easily located, and it accompanies a patient across all healthcare settings. Ideally, the process reflects thoughtful advance care planning conversations between patients and healthcare providers. Research is now demonstrating that the POLST model and form help patients achieve their EOL treatment preferences.

What’s The Missing Ingredient in a Recipe for Effective Healthcare?

People who are food insecure- even at marginal food security- are at greater risk for diabetes, heart disease, stroke, obesity, depression, disability, and premature mortality costing the United States $160 billion in direct and indirect healthcare cost. Food insecurity exacerbates poor glycemic control impacting many patients. Shockingly, 1 in 3 critically ill adults do not have enough money for food, medicine, or both in the United States. Screening for food insecurity and helping patients access important community resources is the missing ingredient in effective healthcare.

A Quadruple Aim and Social Determinants of Health Conversation Starter

It’s no secret that a well-rounded and strategic approach to providing healthcare includes providing treatment for health conditions while trying to prevent health issues that can be avoided. The old adage “an ounce of prevention is worth a pound of cure” is still just as valid and wise today. The IHI Triple aim addresses care, public health, and cost containment. Many organizations have added a fourth aim to address how to take care of healthcare workers specifically. A fourth aim may be named differently, but the fourth usually focuses on taking care of those who take care of others. Some may refer to the fourth as achieving joy, others as improving work-life balance, still others as overall well-being. Regardless, a quadruple aim and an emphasis on the factors in our environments that can improve health equip us with a strategic focus on improving the health of care organizations, healthcare workers, and the public.
