Communication with Patients

Effectively communicating with patients and families is the foundation for a therapeutic, safe, and positive patient experience. The patient‘s experience of care is greatly influenced by what is communicated and what the patient and family observe. Communication is also a vital component of obtaining an accurate history and physical assessment, providing informed, comprehensive care, and educating patients and families to achieve optimal outcomes. When healthcare professionals recognize the factors that influence communication and consistently apply effective communication techniques, patient satisfaction, safety, and outcomes improve.

Preventing Wrong-Site Surgery
We’ve all seen the headlines and heard reports of errors in surgical procedures. These events are shocking and undermine confidence in the healthcare system. Wrong-site surgeries include incorrect site, wrong patient, and wrong procedure, and retained foreign bodies.

Universal Protocol is a Joint Commission strategy for preventing wrong-site surgeries, along with monitoring adherence by accredited facilities. The safe surgery checklist initiative, by the World Health Organization, was established to provide an outline of necessary tasks to complete in each perioperative phase of care. Along with these, there are many other risk-prevention strategies and tools to help keep surgical patients safe from harm.

Every Nurse Is a Risk Manager
Every time you complete an occurrence report for a near-miss medication error or review a clinical policy and procedure with the nurse manager, you are involved in a risk-reduction effort. Healthcare risk management is the process the organization uses to identify, evaluate, and manage risk and potential liability exposure. The objective is for adverse outcomes to be limited by establishing systems to prevent or identify errors before they affect care, and in the event errors occur, to mitigate loss exposure. This module will reinforce and strengthen the understanding nurses have of risk management in healthcare and suggest risk reduction tools to mitigate exposure to professional liability.

The Management of Venous Thromboembolism and Other Clotting Disorders

Hypercoagulability disorder is a highly prevalent clinical condition characterized by abnormal blood coagulation, which leads to thrombosis inside the blood vessels. The most common clinical manifestations are deep vein thrombosis (DVT) and pulmonary embolism (PE); both are types of venous thromboembolism (VTE). DVT may result from the formation of blood clots in the peripheral vascular bed. PE is a life-threatening condition caused by blockage in major pulmonary vessels that rapidly progresses to cardiogenic shock, circulatory failure, and death. VTE can also occur in other sites, including the veins in the brain, kidneys, liver, mesenteric circulation, arms, and legs. The challenge in managing hypercoagulability disorders is due to the clinical variability of the disease etiology and numerous possible sites of blockage balanced against treatment objectives, outcomes, and cost.
